It is not necessary for you to have a doctor’s referral to see a physiotherapist; however, some insurance companies require one for reimbursement. Simply call our office to schedule your in-depth and injury specific assessment.
At Hazelwood Physiotherapy, we provide personalized one-on-one treatments. Our rates:

We accept cash, MasterCard, Visa, Interac (debit), and personal cheque. Payment is required at time of service.
Many extended benefit plans cover physiotherapy services. Please check with your individual provider as to your specific coverage. You will be provided with a receipt that you can submit to your extended medical plan.
In consideration of other patients, we ask that you provide the office with 24 hours notice for any appointment that you must cancel. There is a $25 fee for cancelled appointments within the 24 hour period.
Hazelwood Physiotherapy clinic is open Monday through Friday, 8 am – 8 pm.
Please arrive a few minutes early to fill out a Patient Form and provide details about the problem you want to treat. The physiotherapist will review the form with you, and ask more questions about your injury. The therapist will provide a hands on assessment and treatment during your first visit, and work with you to develop a treatment plan to help you recover.
The length of your appointment will vary depending on your situation. Generally initial appointments will last between 40 minutes to an hour, and all vestibular assessments are 1 hour in length.
Please bring your BC Care Card. If you have a doctor’s referral, ICBC or WCB claim, please bring any related paperwork. Also, if you have a copy of any written reports of x-rays or scans, please bring them to your initial appointment.
If possible, you should wear comfortable and loose clothing that can expose the area that needs to be evaluated and treated (e.g., tank top, pair of shorts). If needed, we also have patient gowns and shorts for you to change into.
There is ample free parking in the parking lot located behind our office building.
